III. Excused absence
a. Taking in consideration, the above mentioned item 9-e in this document,
b. Requests for excused absences from class will be submitted for final approval
using the Excused Absence Request Form. The form will be submitted to both the
course coordinator and Curriculum Central Committee.
c. Approval of all excused absences will be made by the Curriculum Central
Committee and academic advisor. This will be notified to the semester
coordinator.
d. Absence excuse may be accepted in the case of:
i. Physician-documented illness (of more than 2 days) serious enough to prevent
attendance; however absence from a major exam or OSCE due to an illness
must be documented by a student insurance hospital.
ii. Legal proceedings with documentation.
e. If the absence request is approved, arrangements will be made between the student
and course coordinator to administer the graded activity, or a comparable
assessment, at a time convenient for both parties. If the absence request is not
approved, and thus the absence is deemed unexcused, the student will receive a
zero (0) for that graded activity.
f. The request for excused absences should be submitted to the Academic Advisor
via Online or paper based Absence Request form at least two weeks before the
absence occurs.
g. The only exception will be if a personal illness or illness/death of a first degree
family member may not allow time for notification of the Academic Advisor
before the class is missed. However, in those cases, the student should still make
every attempt to notify the Coordinator of the course. However, the student will
still be responsible for submitting the Absence Request Form and necessary
documentation no later than three working days after returning from the absence.
Failure to do so will result in an unexcused absence.
